Patient Eligibility

Our Community

Palliative care, pain control, spiritual and emotional support

Patient eligibility for hospice care is based on the following criteria:

  • The patient has a life-threatening illness, specifically terminally ill cancer, and is seeking medical, spiritual, psychological, and personal support.
  • The patient has decided to discontinue aggressive treatments and instead focus on palliative care, pain control, spiritual and emotional support.
  • The patient has a primary care physician who has authorized hospice care.

Our team of healthcare professionals, including physicians, nurses, and volunteers, work
together to provide compassionate care that meets the unique needs of each patient and their
family. Our goal is to ensure that patients receive the care and support they need to achieve
comfort, dignity, and peace in their final days.
